Kratom: A Southeast Asian Traditional Remedy Adapted to the US Drug Market
When you think of opioids, you probably think of street drugs like heroin or prescription opioids like oxycodone. You might not think of something from a vape shop, corner store, or even a gas station.
Kratom is a natural product, leading many to believe that it’s harmless. It’s a tropical tree native to Southeast Asia; the leaves contain compounds that are pain-relieving medicines in Thailand and Malaysia. It acts as a stimulant in low doses and a sedative in higher doses.
Asians have used Kratom to treat chronic pain for centuries, culturally and under medical advice; to them, it is not a recreational drug but a potent form of medicine.
The Drug Enforcement Administration Finds Kratom Misuse Concerning
The Drug Enforcement Administration (DEA)has published a fact sheet warning the American public about the effects of Kratom addiction.
The DEA warns that Kratom use can cause serious issues, such as psychosis. They also deemed Kratom addictive and noted that the active compounds lead to a high risk for dependency. While Kratom is not yet a scheduled or controlled substance, the DEA continues to educate the public on Kratom misuse and its risks.
Knowing the Potentially Adverse Effects of Kratom Addiction
Abusing Kratom may have impacts on your life that many don’t imagine it could have. From damaging family relationships to spiraling into other mental health conditions, here are some possible adverse effects of Kratom use:
Co-Occurring Disorders and Kratom Misuse
People who turn to substance abuse often have co-occurring disorders. Drug abuse often co-exists with anxiety, depression, or PTSD.
Kratom use can occur alongside mental health conditions. When substance use and a mental health condition co-occur, an integrated assessment can help clinicians address both sets of concerns in a coordinated treatment plan.
The Worsening of Existing Mental Disorders When Using Kratom
People who use kratom may also have co-occurring mental health conditions. Kratom use may complicate symptoms, assessment, medication adherence, and treatment planning.
Kratom can produce psychoactive effects, and responses vary by product, dose, health status, and other substances used. New or worsening changes in mood, perception, judgment, sleep, or behavior should be evaluated by a qualified clinician.
Kratom use may complicate an existing mental health condition, particularly when a person experiences changes in judgment, perception, mood, sleep, or behavior. A qualified clinician can assess possible causes, including substance use, medication effects, and the underlying condition.
Addressing Risk-Taking Behaviors With Appropriate Support
A person experiencing psychosis, severe agitation, dangerous behavior, or an inability to stay safe may need urgent evaluation. If there is immediate danger, call 911 or seek emergency care immediately.
After stabilization, a clinician can recommend an appropriate level of care based on symptoms, safety risks, medical history, current medications, support needs, and the person’s preferences. Prescribed medication should not be stopped or changed without guidance from the prescribing clinician.
Treatment may include medication management, behavioral therapies, skills development, and coordination with other health care professionals when indicated. Services and outcomes vary by person.

Risks of Combining Kratom With Alcohol or Other Substances
Using kratom with alcohol or other substances can make its effects less predictable and may increase risk, particularly when combined with substances that cause sedation. The FDA notes that deaths associated with kratom have usually involved other drugs, although kratom’s contribution in those cases is unclear.
If someone collapses, has a seizure, has trouble breathing, or cannot be awakened, call 911 immediately. For a suspected poisoning without those emergency signs, contact Poison Control at 1-800-222-1222 for case-specific guidance.
Kratom is not FDA-approved to treat pain, mental health conditions, opioid use disorder, or withdrawal. Anyone using kratom to manage symptoms should speak with a qualified health care professional, especially before combining it with alcohol, prescription medication, or other substances.
Pain, sleep problems, and mental health symptoms can have multiple causes. A clinician can evaluate those symptoms and recommend appropriate care without relying on alcohol or unapproved products for treatment.
Choosing an Appropriate Level of Care
An assessment for kratom-related concerns should consider the amount and frequency of use, other substance use, withdrawal symptoms, medical and mental health conditions, current medications, prior emergencies, and available support.
Depending on that assessment, care may include medical monitoring, symptom management, behavioral therapy, substance use treatment, and coordination with other health care professionals. Not everyone needs inpatient treatment, and outcomes vary.
When chronic pain or another medical condition contributes to substance use, behavioral health treatment should be coordinated with qualified medical professionals who can evaluate and treat that condition.
Kratom Binds to the Opioid Receptors in the Brain
Some kratom alkaloids act at opioid receptors and can produce opioid-like effects. Repeated use may lead to tolerance, physical dependence, withdrawal symptoms, or a substance use disorder; effects vary by product, dose, and individual factors.
Opioids can lead to opioid use disorder. Kratom use may lead to a separate substance use disorder and can cause withdrawal symptoms when regular use is reduced or stopped.
Withdrawal Symptoms When You Stop Using Kratom
The withdrawal symptoms are one reason so many turn to Kratom detox centers for help when they want to stop using it. Here are the painful withdrawal symptoms to be aware of before you try to quit:
- Anxiety
- Irritability
- Depression
- Insomnia
- Muscle aches
- Joint pain
- Fatigue
- Sweating
- Nausea and vomiting
- Diarrhea
- Constipation
- Restlessness
- Tremors
- Intense drug cravings for Kratom
These symptoms are moderate for many. But for others, they can be intense and excruciating. It’s best to get help from professional medical detox treatment centers; don’t try to quit cold turkey.
